Where did the expectations for neuropathy, blood sugar, and weight come from?

Alpha-lipoic acid gained attention because it participates in the oxidation-reduction cycle with its reduced form, dihydrolipoic acid. Due to its ability to act in both water and fat, it has been introduced as a 'universal antioxidant.' However, the existence of an antioxidant mechanism does not automatically translate to therapeutic effects for various diseases. It must be remembered first that small changes in a laboratory setting do not necessarily mean a reduction in clinical disease events.

In the past, there were studies involving the short-term intravenous administration of thioctic acid, and these signals led to some research on oral formulations. However, those results cannot be applied directly to general online supplements. This is because the formulation, dosage, administration period, and patient condition in those studies were all different.

NCCIH and Cochrane on Diabetes and Neuropathy

The U.S. NCCIH states that in type 2 diabetes, alpha-lipoic acid was not superior to a placebo in managing blood sugar, cholesterol, and triglycerides, and no evidence was found for the prevention of kidney or macular complications. A 2024 Cochrane review analyzing 3 trials with 816 people treated for 6 months or longer concluded that alpha-lipoic acid is likely to make little to no difference in the symptoms of diabetic peripheral neuropathy.

A 2023 meta-analysis, including shorter oral trials, reported some improvement in total symptom scores. However, nerve conduction, vibration sense, and various neurological indicators did not improve. This means a distinction must be made between symptom relief and nerve recovery. A slight improvement in pain survey scores does not mean that damaged nerves have actually regenerated.

Product Forms: Intravenous and Oral, R-form and Mixtures, Extended-Release

Signals from past studies using 300–600 mg intravenously for 2–4 weeks were based on low methodological quality and medical administration results. An injection given briefly at a hospital and a capsule taken daily at home can produce different results, even if they contain the same substance.

The form is also important. Only the R-form exists in nature, but supplements are usually racemic mixtures of R-form and S-form. There are also extended-release formulations, and in some countries, it is sold as an oral medication. Distorting the evidence occurs when all of these are judged under a single category called 'alpha-lipoic acid supplements.'

Weight, Anti-Aging, and General Fatigue: What has been verified?

Long-term clinical benefits for weight loss, anti-aging, and improvement of general fatigue have not been established. The theory that an antioxidant mechanism exists is on a different dimension from clinical results showing a reduction in diseases or symptoms in actual humans. The fact that small changes in test values do not lead to a reduction in health events applies not only to diabetes and neuropathy but also to other uses.

Precautions when using medications and supplements together

Alpha-lipoic acid may cause hypoglycemia when used with insulin or diabetes medications. You should monitor your blood sugar during use and must not reduce your medication arbitrarily. The EFSA has evaluated that in some people with a genetic predisposition, alpha-lipoic acid supplements can induce insulin autoimmune syndrome, leading to recurrent hypoglycemia. It also noted that a minimum dose that can be definitively called safe cannot be determined.

If hypoglycemia symptoms such as cold sweats, tremors, palpitations, confusion, or fainting recur after intake, you must stop immediately and be evaluated. If you drink excessively, have nutritional deficiencies, have liver or kidney disease, are taking thyroid hormones, are pregnant or breastfeeding, or are undergoing chemotherapy, do not arbitrarily add high doses and consult your treatment team.

In a safety analysis reviewing 71 randomized trials and 4,749 individuals, an increase in overall adverse reactions was not prominent, but it was noted that headaches, heartburn, nausea, vomiting, and skin symptoms may occur.

Why you must first check your individual situation

Then, what should be looked at? First, it must be distinguished whether the cause of the numbness is diabetic polyneuropathy or other causes such as vitamin B12 deficiency, spinal issues, or chemotherapy. If the cause is different, the same supplement carries a different meaning.

These distinctions are the axes that determine the judgment for the same ingredient.

Realistic criteria to check today

In other words, when choosing alpha-lipoic acid, you must first verify the exact identity of the ALA rather than the name 'universal antioxidant'. You must consider whether it is the R-form or a mixture containing the S-form, whether it is sustained-release, whether it is oral or intravenous, what the cause of the neurological symptoms is, how short-term improvement in symptom scores differs from long-term nerve recovery, whether concomitant use with blood glucose or thyroid medications is possible, and whether there are rare signs of hypoglycemia.

Evidence does not simultaneously support the expectations of blood glucose, weight, and nerve recovery. However, the act of distinguishing how much has been verified itself becomes the criterion for selection.
