Lactose in milk meets enzymes to become food for microorganisms
If you take galacto-oligosaccharides, which are food for lactic acid bacteria, will both beneficial intestinal bacteria and constipation improve? In short, some studies show signs of increased Bifidobacteria and improved bowel movement frequency, but it cannot be definitively concluded that this applies to everyone. There is a gap between the expectations created by the name and the actual evidence, due to product forms and individual differences.
When lactose derived from milk meets an enzyme called beta-galactosidase, it can be converted into non-digestible short-chain carbohydrates. This is galacto-oligosaccharide, or GOS for short. The European Food Safety Authority (EFSA) describes GOS as a dietary fiber consisting of 2 to 9 galactose units linked to a terminal glucose, noting that the evaluated raw materials are produced by enzymatically converting milk-derived lactose. Commercial products are often in syrup form, containing remaining lactose, glucose, and galactose, rather than pure GOS. Even under the same name, the actual GOS content and remaining sugars vary by product.
GOS is not the lactic acid bacteria itself
The expression 'food for lactic acid bacteria' often follows when mentioning GOS. However, strictly speaking, this does this something different from 'consuming the bacteria.' GOS is not a living probiotic, but a carbohydrate that reaches the large intestine and is utilized by some microorganisms. It is like providing food for the bacteria, rather than introducing the bacteria directly.
Therefore, GOS and probiotics are often compared. Probiotics involve consuming specific strains of bacteria as they are, while GOS is a fermentable oligosaccharide classified as a prebiotic. While some people take both together, their mechanisms of action are different.
Results confirming an increase in Bifidobacteria are observed
The reason for the expectations surrounding GOS is due to studies related to Bifidobacteria. A meta-analysis of 64 fiber trials reported that fructans and GOS, in particular, were associated with an increase in fecal Bifidobacteria and Lactobacillus. However, overall microbial diversity did not change. In other words, while specific bacteria increased, it does not mean the entire intestinal microbiome became more abundant.
In a trial of 132 adults with self-reported constipation, a daily intake of 11 g showed clear signs of improved bowel movement frequency, particularly in a subgroup with low baseline frequency, and an increase in Bifidobacteria was observed in a dose-dependent manner. However, it is difficult to generalize these results to all participants or to assume the same effect occurs at lower doses.
A 2025 meta-analysis of 20 trials on non-digestible oligosaccharides reported improved bowel movement frequency in the constipation group, but noted that this was not an effect of GOS alone and that the certainty of evidence was low due to heterogeneity and limitations of the studies. This is because GOS, FOS, and inulin are all included under the broad category of 'oligosaccharides,' making it impossible to conclude that the effect was due to GOS alone.
The weight of one packet is not the same as the GOS grams
There is a commonly overlooked point when choosing GOS products. Even if 'GOS' is written prominently on the front, the powder weight of one packet is not the same as the GOS content. Commercial GOS is usually a powdered version of a syrup made by enzymatically converting milk-derived lactose, meaning lactose, glucose, and galactose remain in addition to the pure GOS.
Therefore, you must check the nutrition facts. The total grams of the raw syrup may differ from the actual GOS grams. It is also necessary to check if FOS, inulin, or probiotics are included. If ingredients with similar names are mixed, you may make a choice different from your expectations unless you determine exactly which components are present and in what amounts.

Similar but different ingredients: FOS, Inulin, HMO
GOS is a different ingredient from fructooligosaccharides (FOS), inulin, and human milk oligosaccharides (HMO). While all can be called prebiotics, they differ in chemical structure, the microorganisms they primarily utilize, and the context in which they are used in products. FOS and inulin belong to the fructan family, GOS belongs to the galactose family, and HMOs are complex oligosaccharides found in breast milk.
These must also be distinguished in research. The fact that a meta-analysis of 64 trials showed that fructans and GOS were both associated with an increase in Bifidobacteria only indicates that the two ingredients may act in a similar direction; it does not mean they are interchangeable or have the same effect.
The gap between expectations and symptoms
While results showing an increase in Bifidobacteria can be confirmed, an increase in one type of bacteria or fecal levels does not mean that all outcomes, such as abdominal pain, constipation, and immunity, will improve together. Research only shows signals in specific indicators and does not directly link them to the symptoms an individual feels.
In other words, taking GOS does not mean everyone will feel that their 'gut becomes comfortable.' The fact that bowel movement frequency signals were clear in a subgroup of an adult constipation trial shows that responses can vary depending on the baseline condition. For some, the change is distinct, while for others, it may be minimal or leave only gastrointestinal discomfort.
When used with other supplements or dietary fibers
GOS is often consumed with other prebiotics or dietary fibers rather than alone. In such cases, the total amount of fermentable carbohydrates must be considered. Since gas, bloating, abdominal pain, or loose stools may occur, it is advisable not to start with a large amount at once.
For those suffering from IBS or following a low-FODMAP diet, GOS may worsen symptoms. Due to the nature of fermentable oligosaccharides, the reintroduction amount must be adjusted individually if the gut is in a sensitive state. Another person's appropriate dose is not necessarily the right standard for you.
Individual circumstances and safety signals
GOS is a fermentable oligosaccharide. The EFSA 2025 safety evaluation summarizes that while GOS was generally well-tolerated, temporary gastrointestinal symptoms such as gas and bloating may occur depending on high doses or individual differences.
If you have lactose intolerance, you must check the remaining lactose content in the product; if you have a milk protein allergy, you must separately check the manufacturing ingredients and allergy labels. These two conditions are not the same. Additionally, infants with galactosemia or those requiring special nutritional management should not use general GOS supplements arbitrarily and must follow the instructions of their treatment team or pediatric medical staff.
Realistic criteria to check today
When choosing GOS, it is helpful to ask three things first. First, how many grams of actual GOS are present, not the weight of one powder sachet, and whether the product also contains lactose, sugars, FOS, or probiotics. Second, whether the goal is bowel movement frequency or bloating/IBS symptoms, and whether you are currently on a low-FODMAP diet or using other dietary fibers. Third, whether you have a milk protein allergy, lactose intolerance, or galactosemia, and ensuring that infant products are not confused with adult products.
So, what should you look for? Rather than the name 'food for beneficial bacteria,' a realistic starting point for evaluating this ingredient is to check the actual GOS grams, remaining sugars, complex ingredients, and your desired symptoms, and to record reactions starting from a small amount if you have a sensitive gut.
